Voluntary Self-Identification of Disability Form (CC-305)

Federal contractors and subcontractors have until today, Aug. 4 to incorporate the updated Voluntary Self-Identification of Disability Form (CC-305) into their applicant and human resources systems and processes, according to the Department of Labor, Office of Federal Contract Compliance Programs (OFCCP).

OFCCP acknowledges the updated form will increase the response rate of applicants and employees who voluntarily self-identify their disability status. This will help contractors develop programs to increase the numbers of people with disabilities in the workforce.

OFCCP is committed to providing proactive, meaningful compliance assistance to help contractors solicit disability status information from applicants and employees and meet their equal employment opportunity obligations.
